For many people, using cotton buds (also known as cotton swabs or Q-tips) to clean their ears is a normal ear hygiene practice. It’s satisfying, harmless, and feels like the right way to care for their ears, but this habit can cause more harm than good.
People are often surprised when I tell them that cotton buds are not meant for cleaning the ears at all. They ask questions like, what can be used instead? or what are cotton buds meant for if not for cleaning the ears?
As a doctor, I’ve seen patients come in with ear pain, bleeding, or a cotton bud stuck deep in their ear, simply from trying to clean it. Cotton buds can push wax deeper into the ear, cause injury to the ear canal or eardrum, and increase the risk of ear infections–sometimes leading to hearing problems.
So, why does this happen, and what should you do instead?
In this article, we’ll break down why cotton buds are not safe for your ears, what they are actually meant for, and what you can do instead to keep your ears clean and healthy.
Why Cotton Buds Are Harmful To Your Ears
Cotton buds are harmful to the ears because they push earwax deeper into the ear canal. They can also irritate or cause minor injuries to the canal, which can predispose to ear infections like otitis externa.
Secondly, cotton buds can injure or perforate the eardrum, the membranous tissue responsible for transmitting sound into the middle ear. This can predispose to middle and inner ear infection and hearing impairment.
Cotton buds interfere with the ear’s self-cleansing mechanism to cause more harm than the intended good. Glands in the ear canal produce cerumen (or earwax), which traps dirt, dust, and microorganisms. The skin in the ear canal grows outwards, moving the trapped dirt and wax outwards like a slow conveyor belt.
Jaw movements during talking, chewing, and yawning also help move the earwax outwards without requiring any manual removal. Once wax gets to the ear’s outer opening, it dries up, flakes off, and is washed away during showering.
Cotton buds interfere with this mechanism, push wax back into the ear, and may cause unintended injuries to the ears.
What Are Cotton Buds Actually Meant For?
Though the first Q-tips ‘cotton swabs’ were invented for the delicate cleaning of babies’ ears, they are no longer meant for that purpose. They are currently marketed as safe for external use only.
Cotton buds are used to apply ointments, clean small surfaces, and apply cosmetic makeup. They may also be used by a physician during interventions involving the ears or other body orifices. Longer-sized cotton buds, usually one-sided, are often used for swab sample collection in hospitals and laboratories.
If Not Cotton Buds, What Should You Use Instead?
The simple answer is nothing. The ear has a self-cleansing mechanism for getting rid of dirt and earwax.
If you need to soothe an ear itch, the following may be helpful depending on the cause:
- Ear canal irritation: Apply a warm, damp cloth over the outside of the ear for 5–10 minutes.
- Dry ear: Apply 1–2 drops of olive oil or baby oil into the ear to help moisturize the ear canal.
- Wax impaction: Apply a few drops of hydrogen peroxide twice daily, repeat for 2–3 days until the itching subsides. Alternatively, apply 1-2 drops of olive oil or baby oil into the ear and plug the ear with a cotton bulb, until the wax softens and ear itching or pain subsides.
However, it is best to consult your doctor immediately, as self-treatment can potentially cause more problems than you are trying to solve.
How to Care For Your Ears Safely
Rather than cleaning your ears using a cotton bud, here are dos and don’ts recommended by the World Health Organization (WHO) for caring for your ears without predisposing them to any form of damage.[1]World Health Organization. - Use hearing protection like earplugs for loud music or noisy places: Over time, too loud noise can affect tiny hearing cells of the ears known as the hair cells, predisposing them to hearing problems like tinnitus, vertigo, and hearing impairment.
- Clean the outer part of your ear with a soft cloth: Wax that gets to the outside can easily be removed without using a cotton bud. Cleaning the outer part of your ear regularly helps keep it clean and prevents dirt and debris from entering the ears.
- Only use medicines prescribed by a health worker or doctor: Some medications, including antibiotics, have ototoxic side effects, particularly when used without a doctor’s prescription.
- Go to the doctor or health worker in case of ear pain, discharge, or difficulty in hearing: Some self-interventions and remedies by inexperienced individuals can do more harm than good.
- Avoid Q-tips (cotton buds) and ear candling: Ear candling is an alternative medical practice that is believed to help remove earwax and other impurities from the ear, but this practice is potentially unsafe and non-beneficial to the ears.
- Keep the ears dry.
- Avoid swimming in dirty water.
- Avoid listening to loud earphones or plugging your ears for too long: Plugging the ears for too long can trap moisture
- Avoid sharing earphones or earplugs with others.
- Avoid using ear drops or oils that have not been prescribed by a health worker or doctor.
Common Myths About Ear Hygiene Debunked
Many myths about ear hygiene seem correct, but are not. Understanding these misconceptions can help you avoid habits that can harm your ears rather than keep them healthy.
1. Earwax is dirty and must be manually removed
- Fact: Earwax is not a sign of dirty ears. It protects the ear by trapping debris, dirt, and microorganisms. The ear naturally cleans itself, getting rid of the wax and the trapped dirt and debris.
2. Cotton buds are meant for cleaning the ears
- Fact: Cotton buds are not to be used for ear cleaning. They push earwax back into the ear canal. They can also potentially cause mechanical damage to the ear canal and eardrum.
3. Ear itching is normal and should be scratched when it occurs
- Fact: Itching ears may be common, but it’s not normal. Factors like ear dryness, allergies, wax buildup, mild infections, and irritation caused by cotton swabs or hearing aids. Scratching your ears with any object can worsen the irritation and keep the itching persistent.
4. As long as the object is small, you can insert it into your ear
